The film opens in a prison, where an unseen inmate is being taken to meet with Brigadier Sampson (Jason Watkins). They are conspiring to free the inmate and make it look like they escaped. Sampson asks the inmate if the name “Tewkesbury” means anything to them.

In Malta, Enola Holmes (Millie Bobby Brown) is set to marry Tewkesbury (Louis Partridge). However, she is nowhere to be found at the wedding and is having cold feet. Eventually, Enola decides that she cannot break Tewkesbury’s heart and rides a carriage to the chapel.

En route to the wedding, Enola is pursued by a masked man on a horse. She grabs a rifle from the carriage and tries to fire at the man, believing him to be an assassin, but it is really Dr. John Watson (Himesh Patel). After Enola falls off the carriage, Watson tells her that Sherlock (Henry Cavill) has been kidnapped.

Enola narrates the events that led her to end up in Malta. Tewkesbury had been moving up among the House of Lords, while Enola has become recognized as a noteworthy detective, alongside her brother. The two would occasionally work together, with Enola hoping to use Sherlock’s resources to help the less fortunate. Tewkesbury had proposed to Enola, but Sherlock could not express excitement due to his obsession with a mystery that was puzzling him. During their engagement party, Sherlock and Enola got into an argument over her getting married, and she disowned him as her brother.

Enola joins Watson at the scene of the kidnapping. Sherlock had left a message on the mirror in morse code that Enola translates as “Khost”, suggesting the Battle of Khost in Afghanistan has some part in the mystery. Enola goes through Sherlock’s room and finds clues, like war medals and a piece of a dress.

On the streets, Enola spots a Maltese sergeant and pursues him. When she corners him, he is shot by an unseen assailant. He can only utter what sounds like “wrath” before he dies. Later, Tewkesbury visits Enola, who apologizes to him for leaving him at the altar and explains her situation.

Enola later finds Tewkesbury’s hotel on fire, with him having gone inside to rescue his mother (Hattie Morahan). Enola runs inside and deduces that Lady Tewkesbury was kidnapped, so she gets Tewkesbury out of the burning building.

While trying to solve the case on her own, Enola meets Mikiel Mizzi (Joe Azzopardi). They engage in brief combat when she thinks he might be involved in Sherlock’s disappearance, but Mikiel suggests that someone named Professor Adeline Rathe is responsible. He claims that Rathe is dangerous and has “the governor’s ear”. Mikiel flees when the police arrive.

Enola travels to Valletta after tracking the dress piece there. She spots a mysterious woman on the streets speaking to the governor, and follows her into the sewers, believing she is Rathe. Before Enola can confront the woman, she is shot by the same unseen assassin. Enola keeps walking until she realizes that “Adeline Rathe” is really none other than…MORIARTY (Sharon Duncan-Brewster). The villain attempts to hang Enola but she fights her way out. Their scuffle is interrupted by Enola’s mother Eudoria (Helena Bonham Carter), but Moriarty escapes after setting off an explosive. Before parting ways again, Eudoria encourages her daughter to think like Sherlock when solving the case. Enola later joins Tewkesbury for a romantic afternoon together.

Moriarty is revealed to be keeping Sherlock and Lady Tewkesbury imprisoned. She makes Sherlock try to figure out what her true plan is, even keeping water from him and Lady Tewkesbury to be cruel.

Later, Enola meets with Tewkesbury, Watson, Eudoria, and Mikiel. Mikiel is revealed to be part of an anti-reformist organization that wants to abolish the British Empire’s influence in Malta. Tewkesbury also suggests that the woman Enola encountered was Moriarty’s spy. Enola points out how the woman was talking to the governor, as well as Sampson.

The group gets further into the investigation as Tewkesbury and Watson meet with Sampson to learn about the connection to Khost, while Enola, Eudoria, and Mikiel sneak around to find private documents. Enola finds a message written by the sergeant, who was Moriarty’s inside man. After retracing it, Enola reads it, which mentions a secret mission in Khost involving one ton of gold that was smuggled to Malta, as well as numerous deaths of crew members. Enola goes to tell Tewkesbury about it, and Sampson confirms that Tewkesbury’s father was the one who ordered the mission and blamed Maltese soldiers for the casualties and “lost” gold. The war medals also belonged to Lord Tewkesbury. Tewkesbury is disheartened to learn the kind of man his father really was.

Enola gets all the info she’s learned and realizes the missing gold is hidden in a cave. She knows that Moriarty organized every single thing to get them there, down to setting her and Tewkesbury up to be married in Malta specifically. With help from Watson, Enola tracks the cave down and finds the gold. Unfortunately, she and Watson are found by Moriarty’s henchmen, as well as the villain herself, who is acting against the British Empire. Tewkesbury and Mikiel show up with reinforcements, Mikiel’s fellow fighters, who take on the henchmen while Moriarty grabs the gold and tries to escape.

Enola chases after Moriarty to the location she is keeping Sherlock and Lady Tewkesbury. Enola fights her nemesis one on one before knocking her out. She finds the captives and frees Sherlock, but Moriarty attempts to kill them both. After she is once again subdued, Sherlock prepares to shoot Moriarty, knowing that if she is arrested that she will just keep coming after their family, but Enola convinces him not to be a murderer. Lady Tewkesbury frees herself and whacks Moriarty over the head.

Moriarty is sent to a higher security prison, while Tewkesbury arranges for the stolen gold to be sent back to where it belongs. Sampson willingly surrenders and is imprisoned. Enola and Sherlock later reconcile.

Enola and Tewkesbury share a moment together at night, where he tells her that he is dropping his father’s name and title so that he may go by his real name of “Earnest Augustus Tebbity-Gore”.

The wedding goes on ahead as planned, with Eudoria officiating and even inviting Enola’s mentor Edith (Susan Wokoma) as a surprise guest. Enola and Tewkesbury are married. As they spend time in the water, we move down below the surface to discover the sunken ship, “The Wrath of Adeline”, which is where the gold came from and how Moriarty came up with that alias.
